Initial enquiries are welcomed from various sources commissioning authorities, social services, solicitors, Court and directly from parents. They can be made by telephone, email, fax or letter.
Enquiries will be responded to by a senior member of the team who will provide information about the service requested and the referral process.
A referral form can be downloaded from our website which then can be either emailed or posted direct. The document needs to be completed in full and supporting information provided.
A senior panel will meet following receipt of the information to decide whether or not the service can meet the needs of the family. Once agreed a start date will be allocated.
The family will be invited to meet with the staff prior to the assessment or service commencement. This will enable parents to find out more about the process, meet their allocated case manager where applicable, make an informed decision and sign any relevant forms.
